Lumen Healthcare provides high-quality home care, live-in care and complex nursing support to adults in their own homes.

Our services support adults aged 18 and over, including older adults, people with dementia, learning disabilities, autism, neurological conditions, physical disabilities and individuals with complex healthcare needs.

Our complex care services may include tracheostomy care, ventilation, suctioning, oxygen therapy, enteral feeding, epilepsy management, catheter care, stoma care, complex bowel care, diabetes management and other delegated clinical interventions.

We are recruiting an experienced Registered Nurse to join Lumen Healthcare as Care Coordinator / Clinical Lead.

About the Role

The Care Coordinator / Clinical Lead will provide clinical leadership across Lumen Healthcare's home care, live-in care and complex care services.

The successful candidate will be responsible for coordinating care packages, completing clinical assessments, developing and reviewing care plans, assessing staff competency, monitoring quality and supporting nurses and support workers to deliver safe, effective and person-centred care.

This is a hands-on clinical leadership role that combines care coordination, clinical governance, staff supervision and quality assurance.

Main Responsibilities

Complete initial and ongoing clinical assessments for new and existing service users.

Develop, implement and regularly review person-centred care plans, risk assessments, emergency plans and clinical protocols.

Coordinate complex home care and live-in care packages and ensure appropriate staffing is maintained.

Assess, supervise and sign off staff competency for delegated clinical procedures.

Provide clinical leadership and advice to Registered Nurses, support workers and care staff.

Undertake care reviews, spot checks, competency assessments, supervision and quality audits.

Monitor medication administration and undertake MAR or eMAR audits.

Review incidents, medication errors, safeguarding concerns, complaints and near misses and ensure appropriate action is taken.

Support investigation of incidents and identify learning, corrective actions and improvements.

Ensure care is delivered in accordance with current legislation, CQC requirements, company policies and professional standards.

Liaise with service users, families, commissioners, NHS teams, local authorities and multidisciplinary professionals.

Ensure staff are appropriately trained and competent before providing complex care independently.

Support recruitment and clinical assessment of Registered Nurses and complex care support workers.

Contribute to staff induction, training and continuing professional development.

Support the Registered Manager with clinical governance, quality assurance and service improvement.

Participate in the on-call or escalation arrangements where required.

Clinical responsibilities

The successful applicant should have strong experience in complex care and ideally be competent in several of the following areas:

Tracheostomy care.

Tracheal suctioning.

Invasive and non-invasive ventilation.

Oxygen therapy.

Cough assist.

Nebuliser and inhaler therapy.

PEG, gastrostomy and jejunostomy care.

NG feeding.

Enteral feeding pumps.

Medication via feeding tubes.

Epilepsy management.

Catheter care.

Stoma care.

Complex bowel management.

Diabetes care.

Wound and pressure-area management.

Neurological conditions.

Acquired brain injury.

Spinal injury.

Palliative and end-of-life care.

Applicants do not need to be competent in every specialist area, but must demonstrate strong clinical judgement and the ability to safely assess and oversee complex care.

Essential qualifications

Registered Nurse with current NMC registration.

Minimum of three years' post-registration nursing experience.

Strong experience in complex care, critical care, intensive care, community nursing, home care or another relevant clinical setting.

Experience assessing clinical needs and developing care plans.

Experience with risk assessment and clinical decision-making.

Experience supervising or supporting healthcare staff.

Ability to assess staff competency and identify unsafe practice.

Strong understanding of safeguarding adults.

Strong understanding of the Mental Capacity Act 2005 and consent.

Strong medication management knowledge.

Excellent written and verbal communication skills.

Ability to maintain accurate clinical documentation.

Ability to work independently and make sound clinical decisions.

Good organisational and time-management skills.

Right to work in the United Kingdom.

Willingness to undertake an Enhanced DBS check.

Desirable qualifications and experience

Previous experience as a Clinical Lead, Care Coordinator, Complex Care Nurse or similar role.

ICU, HDU or complex care experience.

Experience supporting adults with learning disabilities or autism.

Experience conducting clinical audits.

Experience with CQC-regulated services.

Experience investigating incidents and medication errors.

Leadership or management qualification or equivalent

Practice assessor, mentorship or teaching qualification.

Safeguarding training at an appropriate level.

Knowledge of electronic care-planning and eMAR systems.

Full UK driving licence.

Access to a vehicle.
