Pay Rate: Competitive – DOE (Band 7–8a equivalent)

Job Description

Eden Brown Synergy is currently supporting a leading NHS organisation in Central London seeking an experienced Clinical Pharmacist to join their Primary Care Team across GP surgery sites in Hammersmith and Charing Cross.

This is an excellent opportunity for an experienced pharmacist to work within a supportive, multidisciplinary primary care team, contributing to safe and effective medicines management and improving outcomes for patients with complex or long–term conditions.

Key Responsibilities

Undertake structured medication reviews for patients with chronic or multiple long–term conditions.

Provide expert advice on medicines optimisation, safety, and cost–effective prescribing.

Support GPs and nursing staff in reviewing repeat prescriptions, reducing polypharmacy, and preventing adverse interactions.

Assist in the management and monitoring of high–risk medicines.

Contribute to clinical audits and quality improvement initiatives within the practice.

Deliver patient counselling and education on medication adherence and self–management.

Ensure compliance with NICE guidance, CQC standards, and local prescribing policies.

Support implementation of national and local medicines management initiatives.

Collaborate closely with GPs, Practice Nurses, and other members of the Primary Care Network (PCN).

Requirements

GPhC–registered Pharmacist (essential).

Previous experience within a GP or primary care setting (essential).

Strong background in medication reviews and medicines optimisation.

Excellent understanding of prescribing systems and clinical governance.

Independent Prescriber qualification desirable but not essential.

Familiarity with SystmOne desirable but not required.

Excellent communication, teamwork, and documentation skills.
