As Children's Continuing Care Coordinator, you will play a vital role in ensuring children and young people with complex health needs receive timely, effective and coordinated support through the Children's Continuing Care process.
Working within the Children and Education Brokerage and Placements Team, you will provide specialist oversight and coordination of Children's Continuing Care referrals, supporting children, young people and families through every stage of the pathway. This includes identification, triage, referral, assessment, panel presentation, funding agreement, monitoring and review activities.
You will work closely with colleagues across social care, health, education, finance, commissioning, providers and the Integrated Care Board (ICB) to ensure assessments and funding arrangements are robust, timely and centred on the needs of children and young people. You will be responsible for coordinating information, tracking progress and supporting effective decision-making to ensure that care packages are delivered efficiently and in line with national guidance and local procedures.
The role requires strong partnership working, attention to detail and a commitment to improving outcomes for children and young people with complex needs. Through effective coordination and case oversight, you will help ensure children receive the right support at the right time.
