Support Worker

Description

We are looking for a number of experienced practitioners within Evesham/Pershore to deliver trauma-informed, evidence-based support to adults affected by substance misuse. This role involves managing a complex caseload, conducting specialist assessments, care planning, and delivering psychosocial and clinical interventions. You’ll work closely with multi-agency partners to improve clients’ health, resilience, and social outcomes.

Key Responsibilities:

Carry out comprehensive assessments and deliver individual and group interventions focused on substance misuse recovery.

Support clinical activities including prescribing, drug screening, and physical health monitoring.

*

Develop and review personalised care plans to address wider health and social needs.

*

Manage risk effectively and ensure safeguarding standards are upheld.

*

Collaborate with partner agencies to ensure coordinated, holistic care.

*

Lead and participate in community outreach and targeted awareness campaigns.

Candidate Profile:

*

Proven experience in mental health and support

*

Strong communication and relationship-building skills to engage vulnerable adults.

Ability to work flexibly to meet client needs, including occasional evening work.
