Temporary, Ongoing Assignment 40 Hours per Week

We are recruiting a Prescribing Pharmacist for a fully remote role supporting an innovative Hospital-at-Home service. This service enables patients to safely receive care and monitoring in their own homes through digital healthcare solutions, improving outcomes while reducing hospital admissions.

About the Role

As a Remote Prescribing Pharmacist, you will conduct telephone-based medication reviews with patients enrolled in the service. You will focus on optimising medication regimens, identifying issues early, and making prescribing decisions where clinically appropriate.

This is a temporary, ongoing position, with 40 hours available per week.

Key Responsibilities

Conducting structured medication reviews with patients over the phoneReviewing current medications for safety, effectiveness, and adherenceCalling patients to discuss symptoms, side effects, and treatment outcomesAmending, initiating, or stopping medications in line with clinical guidelinesClearing and managing routine medication-related alertsIdentifying red flags and escalating concerns to nurses or clinicians where requiredDocumenting all consultations and changes accurately on clinical systemsProviding clear advice, reassurance, and patient educationWhat We're Looking For

Registered Pharmacist with Independent Prescribing qualificationExperience conducting medication reviews (primary care, community, or remote settings)Strong clinical decision-making and risk assessment skillsConfident telephone consultation mannerAbility to work autonomously within a remote clinical environmentExcellent documentation, organisation, and communication skillsComfortable using digital systems following trainingWhat's on Offer
