Experienced Occupational Therapist – Reablement
Location: South West England
Contract: Full Time
Sector: Adult Social Care
An experienced Occupational Therapist is required to join a busy Reablement Team within a local authority Adult Social Care service. The team focuses on supporting individuals to regain independence and reduce the need for long–term care.
Key Responsibilities
Manage a complex caseload of adult clientsComplete occupational therapy assessments and develop reablement plansCarry out moving & handling and risk assessmentsRecommend equipment, adaptations and Disabled Facilities GrantsWork collaboratively with health professionals, families and partner agenciesProvide professional support and guidance to colleaguesMaintain accurate records and reportsRequirements
Degree or Diploma in Occupational TherapyHCPC registrationMinimum 2 years post–qualification experienceExperience working with adult clients and complex needsKnowledge of health, housing and social care legislationExperience with adaptations, reablement and moving & handlingAdditional Information
