Health and Wellbeing Practitioner – HMP Send

Location: Surrey

Salary: GBP27,857.14 per annum

We are looking for a Health and Wellbeing Practitioner to join The Forward Trust at HMP Send, Surrey, working full–time, 37.5 hours per week.

In this role, you ll deliver clinical and psychosocial substance misuse services within a trauma–informed, recovery–focused framework, supporting female service users with substance misuse and low–level mental health needs. You ll help them build healthier futures both inside the prison and as they transition back into the community.

What you ll be doing:

Manage your own caseload, delivering 1:1 and group interventions tailored to individual needs Conduct assessments, co–produce care plans, and provide harm reduction advice Facilitate release planning and through–care support for service users returning to the community Work collaboratively with multi–disciplinary teams across mental health, healthcare, and the prison Ensure accurate, timely recording of outcomes and data to track the impact of your work About you:

All prison–based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks.
