Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing care services, ensuring clients receive personalised support tailored to their individual needs. This position offers an opportunity to make a meaningful difference in people's lives while working within a supportive and professional environment. Prior care coordinator experience is a MUST, as it will aid in efficiently managing administrative responsibilities associated with the role.
Duties
Coordinate and schedule care plans for clients, ensuring timely and appropriate support. Liaise with healthcare professionals, clients, and their families to facilitate effective communication and service delivery. Maintain accurate and up–to–date records of client information, care schedules, and service documentation. Provide administrative support including managing appointments, preparing reports, and handling correspondence. Monitor client progress and adjust care plans in collaboration with relevant stakeholders. Ensure compliance with organisational policies and relevant health and safety regulations. Assist in the onboarding of new clients and staff members as required. Taking on the responsibility of having the on–call and ensuring all calls are answered and actioned in a professional manner. Requirements
Previous Care Coordinator experience is essential to effectively manage administrative tasks. Excellent organisational skills with the ability to prioritise tasks efficiently. Strong communication skills, both written and verbal, to liaise effectively with clients, families, and healthcare professionals. Compassionate approach with a genuine desire to support individuals in need of care. Ability to work independently whilst also being a team player. Good computer literacy, including familiarity with standard office software programmes. Knowledge of Birdie – Monitoring Software Flexibility to adapt to changing priorities within a dynamic environment. Able to work evening , weekdays and bank holidays
