Adult Social Worker – 0–25 Transforming Care Team

Location: Hertfordshire (Countywide)

Pay: 32 per hour

Role: Adult Social Worker – 0–25 Transforming Care Team

About the Role:

We are seeking an experienced Adult Social Worker to join the 0–25 Together Service within the Transforming Care Team. This role focuses on supporting young adults aged 18–25 with complex mental health needs who are at risk of hospital admission.

You will work both remotely and from office locations across the county, so a full driving licence and access to a car are essential. Due to the nature of the work, we require a female social worker to support individuals living with trauma who may be triggered by male workers.

Key Responsibilities:

Complete Care Act and Capacity assessments.Undertake Community DoLs applications and prepare reports for court.Support transitions in and out of hospital admission.Carry out risk assessments and work within a multi–agency team including mental health services, learning disability teams, schools, colleges, and providers.Apply knowledge of CETR, DSR, and the Mental Health Act.What We're Looking For:

Qualified Adult Social Worker with current registration.Experience in adult safeguarding and working with learning disabilities and transition cases.Strong understanding of complex mental health needs and risk management.Flexible and able to work under pressure.Why Join Us?
