Description

On behalf of a group of friendly, forward–thinking practices in Plymouth, an exciting opportunity has arisen for an experienced Advanced Nurse Practitioner (ANP) to join their well–established primary care team.

The practices are committed to delivering high–quality, patient–centred healthcare to their local communities and are now seeking a motivated and skilled ANP to strengthen their clinical offering. The successful candidate will be an autonomous practitioner who can assess, diagnose, and manage a wide range of patient presentations. They will be confident in independent prescribing and skilled at delivering same–day care, as well as managing chronic conditions, undertaking clinical examinations, and promoting health and wellbeing. Working closely with GPs, practice nurses, and allied health professionals, the ANP will play a key role in ensuring patients receive safe, effective, and compassionate care.

Key Responsibilities:

Undertake patient assessments, examinations, and diagnostic decision–making

Prescribe and review medications in accordance with local and national guidelines

Provide care for patients with undifferentiated and undiagnosed conditions

Manage both acute presentations and long–term conditions

Contribute to the delivery of enhanced services and clinical pathways

Work collaboratively with colleagues across the practices and wider network

Support health promotion, prevention, and patient education initiatives

Actively engage in clinical governance, audit, and service development

Essential Requirements:

Registered Nurse with an MSc or equivalent in Advanced Clinical Practice

Independent prescribing qualification (V300)

Demonstrable experience in primary care, urgent care, or a similar setting

Excellent clinical decision–making and problem–solving skills

Ability to work independently as well as within a multidisciplinary team
