Job Title: Social Work Assistant

Directorate: Social Services

Section: Assessment and Case Management (Adult Services Review Team)

Location: Cardiff

Grade: 5

Pay Rate: £16.62 per hour

Hours: 37 per week (09:00 – 17:00)

Reporting to: Team Manager

Contract Period:

Start Date: 02/03/2026

End Date: 01/06/2026

Job Purpose

To support the Adult Services Review Team in delivering high-quality assessment, review, and case management services to older people and adults with physical disabilities. The post holder will work closely with the Team Manager, Social Workers, and Duty Workers to ensure care plans are effectively implemented and reviewed.

Key Responsibilities

*

Arrange and monitor services in line with individual care plans.

*

Provide advice and practical support to service users and carers.

*

Liaise with statutory and voluntary agencies.

*

Complete annual care plan reviews and reassessments.

*

Participate in team meetings and duty rotas.

*

Work in accordance with Council policies, procedures, equality, and health & safety requirements.

*

Engage in ongoing training and professional development.

Essential Requirements

*

GCSEs (or equivalent).

*

Experience working directly with older people or adults with physical disabilities.

*

Strong communication, organisational, and administrative skills.

*

Ability to work independently and as part of a team.

*

Commitment to equality and supporting vulnerable adults.

Desirable:

*

NVQ Level 3 or 4 in Social Care. xjnqpsq

Experience in multi-agency or hospital settings.

*

Full valid driving licence and access to a vehicle
