Outreach Worker – Rough Sleeping Team

Location: North West London

Rate: £22.38 per hour (umbrella)

Contract: 3-month ongoing contract

We are looking for a compassionate and proactive Outreach Worker to join the Rough Sleeping Team in the Council, supporting the borough’s operational and strategic response to rough sleeping. This is an excellent opportunity to make a tangible difference in people’s lives by helping vulnerable individuals access housing and support services.

Role Purpose The Outreach Worker will act as a single point of contact for rough sleepers with low to medium support needs, working closely with partners across the statutory and voluntary sectors. You will help individuals move into suitable accommodation and provide the support they need to sustain it, reducing repeat rough sleeping.

Key Responsibilities

Work collaboratively with the Council’s homelessness partners to deliver an effective response to rough sleeping.

Support rough sleepers with low to medium needs, creating and implementing individualised action plans.

Work closely with the Rough Sleeper Co-ordinator and Single Homeless Officer to ensure joined-up support and progression toward sustainable housing solutions.

Ensure individuals engage with relevant support services, including health, substance misuse, and employment support.

Provide guidance and ongoing assistance to help clients maintain emergency or temporary accommodation and transition successfully into long-term tenancies.Requirements

Experience working with rough sleepers or vulnerable adults within housing, homelessness, or support services.

Strong understanding of homelessness pathways, safeguarding, and partnership working.

Excellent communication and case management skills.

Empathetic, resilient, and able to work independently in a fast-paced environment.Benefits
