About the Client
Jacob is 16 years old, and lives at home with his parents and younger brother in Woking. He is an engaging, funny, charming young man with a great sense of humour. There is a cheeky side to his personality as he loves to joke around or play pranks. He loves to chat and is a great conversationist! Jacob is an avid Football Fan, and travels to watch games; this is a big hobby! Another passion of his is music, any genre at all, anything with a beat! Other hobbies include gaming, swimming, cooking and skiing. Jacob has Cerebral Palsy, and because of his condition he requires practical support to meet his care needs. He is a wheelchair user, and can weight bear, and transition with support (equipment to assist such as a Sara Steady). There is a lift in his home that Jacob uses for getting up or downstairs. All in all, Jacob is a really cool guy, and a typical teen, and he is excited to expand his support team to facilitate his independence.
The Role at a Glance
The afternoon shift starts promptly at 4.30pm as Jacob arrives home from school. Once at home, Jacob will usually have a snack, a bit of a chill, or therapy, depending on which day it is, dinner, bath and bed. He will let you know what the plan is, or what he would like to do for the afternoon / evening. There is more scope during school holidays to plan activities, day trips etc. Jacob’s night carer will arrive at 8.00pm and a full handover is complete.
