Are you an experienced Clinical Nurse Specialist, Palliative Care Nurse, Hospice Nurse or Community Nurse looking for a role where you'll have the autonomy, support and time to deliver truly exceptional end of life care?
Join a specialist palliative care service supporting patients with life–limiting illnesses across Berkshire West and South Oxfordshire. Working alongside an experienced multidisciplinary team, you'll provide expert clinical care that helps patients remain comfortable, supported and in control, while making a lasting difference to the people closest to them.
Based from one of the Berkshire hubs, you'll work across local services throughout Berkshire West and South Oxfordshire, combining autonomous clinical practice with close multidisciplinary collaboration.
The Role
As a Palliative Care Nurse, you'll provide specialist assessment, expert symptom management and high–quality palliative care for patients with complex needs.
Your responsibilities will include:
Delivering holistic assessments and developing individualised care plansManaging complex symptoms and supporting patients approaching the end of lifeProviding specialist advice and guidance to patients, carers and healthcare professionalsSupporting telephone triage and responding to clinical enquiriesWorking collaboratively with GPs, community teams and the wider multidisciplinary teamActing as a patient advocate, ensuring compassionate, person–centred care at every stageMaintaining accurate, high–quality clinical recordsAbout You
You'll be an experienced clinician who enjoys working autonomously while remaining part of a supportive specialist team.
You'll have:
RGN with active NMC registrationPrevious palliative care experienceFull UK driving licence and access to your own vehicleExcellent communication, assessment and clinical decision–making skillsA genuine passion for delivering outstanding palliative and end of life careIf you haven't yet completed your Non–Medical Prescribing qualification, you'll receive full support to achieve this as part of your ongoing professional development.
