Description

Family Hub Worker

We are looking for an experienced Family Hub Worker to join an Early Help service supporting children, young people and families across Trafford. Working as part of the Family Hub model, you will provide early intervention and practical support to help improve outcomes for families, ensuring children are safe, supported and able to thrive.

Key Responsibilities Provide early help support to children, young people and families using a whole–family approach. Manage a caseload as a Key Worker, coordinating assessments, support plans and interventions. Work closely with partner agencies including schools, health professionals, early years providers, housing and police. Deliver one–to–one and group programmes for children, young people and parents. Support families with emerging developmental needs and SEND. Maintain accurate records and monitor the impact of interventions to achieve positive outcomes. Requirements Level 3 qualification in a relevant subject (or equivalent experience). Experience working with children, young people or families within early years, education, social care, health or SEND services. Strong partnership working, communication and case management skills. Full UK driving licence and access to a vehicle.
