Hospital Discharge Administrator Hospital Discharge Administrator Location: Manchester-sited within one of the three hospital sites in Manchester (North Manchester General, Manchester Royal Infirmary, Wythenshawe Hospital) Hours: 35 per weekContract: Immediate start up to March 2026Salary: 24, 412 per annumBasic DBS check requiredWe're looking for a compassionate and organised Project Administrator to support carers during hospital discharge. You'll be the vital link between hospital teams and community services, helping carers access timely support and preventing carer breakdown. This role is key to ensuring smoother, safer transitions from hospital to home.What You'll DoBuild relationships with hospital staff to promote carer recognition and involvement in discharge planningSupport a caseload of carers through the discharge processConduct Carers Assessments and refer individuals to appropriate servicesDeliver training and awareness sessions to hospital teamsCollaborate with the Manchester Carers support team and contribute to monitoring and reportingWhat We're Looking ForWe're seeking someone who is:Experienced in supporting vulnerable individuals and understanding carers' needsKnowledgeable about safeguarding, statutory services, and community care structuresProficient in Microsoft 365 and client data systemsHighly organised, with excellent communication and interpersonal skillsCommitted to equity, diversity, and inclusionDesirable ExtrasDegree-level education or equivalent experienceExperience delivering training and conducting statutory Carers AssessmentsFamiliarity with Greater Manchester's VCSE sector and carer support landscapeWhat you need to do nowIf you're interested in this role, please forward an up-to-date copy of your CV.
