Job Title: Care Coordinator

Salary: Up to 32,000 PA DOE

Location: South London

This is an excellent opportunity for an experienced care coordinator to join a leading independent home care provider within their South London branch and support the delivery of personalised, high–quality self–funded care.

Our client deliver up to 2000 hours of care per week, and have recently been inspected as Outstanding by CQC.

Requirements

Ideally NVQ Level 2 or 3 Previous experience within a scheduling roleKnowledge of CQC compliance is essentialGood working knowledge of IT SystemsIdeally experienced in conducting care assessments, care reviews and being able to motivate care staff communicate effectively and efficiently via phone, email and in–person meeting

Responsibilities

Day to day management of schedules around caregivers and clients.Working with clients and their familiesEnsure care delivered is compliant with all relevant legislationUndertake assessments and reviewsUndertake supervisions and direct observationsImproving and maintaining the continuity of care

Remuneration and Benefits:

Up to 30,000 PA DOEFlexible working25 days holiday + bank holidays
