We are looking to recruit a Care Co-ordinator for our Domiciliary Agency, Longdene Homecare, which is part of the CHD Living group.

You will be required to co-ordinate and maintain continuity of care and support to Service Users and to ensure efficiency and reliability of all visits. To be dedicated, willing to work in a busy team environment, have strong interpersonal skills, being proactive and organised in nature. To support the Branch Manager in dealings related to clients, staff and outside organisations

Minimum Requirements

Health & Social Care Diploma (Level 3)
Full driving licence and own car.
Processing of all new business referrals, including visits to new clients for the purpose of carrying out full assessments.
Rostering, spot checks and supervision, care assessments and reviews.
On-call: One weekend in four and one week day.

Successful Applicant will be subject to references and an enhanced CRB disclosure (expense met by employer). You will also be enrolled on the Simply Health scheme on successful completion of probation. We are an equal opportunities employer.

Salary: £17,500 per annum
Hours 42 per week
