Band 6 Response/ Enablement Occupational Therapist

The Client Provides A Service That Provides Home Based Support And Rehabilitation For Patients Aiming To Maximise Independance. Patients Will Be 18 Years+.

OT Will Be Carrying Out Assessments And Rehabilitation Working Within A Multi-disciplinary Team.

Role To Consist Of Enablement, Supported Discharge And Rapid Response.

Occupational Therapist Will Be Facilitating Timely Discharge Home From Hospital In Order To Maximise The Potential For Recovery And Minimise Readmission To Hospital Or Long Term Institutional Care, Timely Intervention To Avoid The Need For Hospital Admission.
