Qualified Social Worker
A unique opportunity has arisen for a Qualified Social Worker for Older People Team in South East London.
Responsibilities
1. Assess the needs of individuals and their carers.
2. Together with the service users and carers, formulate and implement care plans appropriate to their needs.
3. Provide counselling and continuing support to users and carers where appropriate.
4. Monitor, review and evaluate services in order to ensure that they are provided effectively and in accordance with need.
5. Participate in the operation of duty and respond accordingly to need.
6. Provide an assessment service to individuals, their carers and other agencies in accordance with fair access to care eligibility criteria. These cases are likely to fall within the critical and substantial bandings.
7. Initiate and co-ordinate care plans for and with individual service users and their carers, in conjunction with professional colleagues and representatives of voluntary and private agencies.
8. Supervise and support staff as agreed with Line Manager.
9. Offer support and advice as appropriate to service users and their carers.
10. Through the assessment and prioritisation of need, purchase appropriate services and authorise expenditure on services within designated limits.
Pay rate is between £20 and 24 per hour.
GSCC Registration is essential.
