Senior Practitioner
We are currently seeking a Qualified Senior Practitioner with a Leaving Care Team in the North West London Area.
Responsibilities:
1. To hold ongoing key - worker responsibility for a caseload of varying complexity of children and young people who are -in need', looked after by the council on the child protection plan or the subject of care packages.
2. To undertake assessments and to recommend to the team manager the appropriate level of intervention and of service position
3. To work creatively and to produce plans for children, who are in need looked after or by the child protection register
4. To implement and review these plans
5. To keep cases records up to date and to carry out other administrative duties as required
6. To take appropriate steps to maintain satisfactory level of security and confidentiality of all records
7. To chair or participate in reviews and meetings attended by the users as well as by colleagues and representatives of other agencies e.g. Health, Education, Probation and police
8. To develop and maintain effective working relationships at an operational level with relevant statutory and independent sector agencies as stated above.
Pay rate is up to £27 per hour
GSCC Registration is essential.
